Provided in the links below is information regarding sports physicals. The first link is a copy of the Pre-Participation Physical Evaluation Form (valid for 2 years), the second link is the MSHSAA Pre-Participation Documentation - Annual Requirements, and the third is the COVID Waiver Form.
The only document that requires seeing a doctor is the Pre-Participation Phsyical Evaluation Form. All others may be completed at home. The MSHSAA Pre-Participation Documentation - Annual Requirements was updated in June of 2020 so make sure to use this document instead of any older copies you may have.
If your student had a 2-year physical completed last year, all that would need to be filled out this year would be the MSHSAA Pre-Participation Documentation - Annual Requirements Form.
- Pre-Participation Physical Evaluation Form (valid for 2 years)
- MSHSAA Pre-Participation Documentation - Annual Requirements Form
- COVID Waiver Form
A valid Pre-Participation Physical Evaluation Form, MSHSAA Pre-Participation Documentation - Annual Requirements Form, and COVID Waiver Form must be on file with the school before any student will be allowed to participate this summer and/or during season.
